AHA was appointed by the Victorian Government to undertake a three-year review of the implementation of the Active Service Model (ASM).

ASM PREPARE practice review and planning tools were developed to assist HACC funded agencies to:

• Develop a structured view of their current strengths and areas for improvement in delivering an ASM approach
• Identify current practices that were already consistent with an ASM philosophy of care
• Identify changes in structure and practice required to move to an ASM.

AHA’s three-year review involved:

• Review of operation and use of online tools for ASM PREPARE, to identify progress by funded organisations in implementing the ASM approach
• Targeted workshops with a sample of agencies, to identify enablers and barriers to ASM approach implementation and to identify priority areas and actions for further implementation of the ASM
• Final report of key findings from the review process, and identification of priority areas and actions to assist Agencies and the department to further embed the ASM approach.
